Dr. Melendez accepts the following health insurances:
- Aetna
- United Healthcare
- Oscar
- Oxford in the states of Oregon and Florida.
Sessions are 45 to 50 minutes long, depending on the client’s needs and what your insurance covers.
Dr. Melendez uses a video system integrated with Simple Practice. You will receive a link to join your session or access the session from your client portal at the time of your appointment.
- Initial Intake: $200 for a 60 minute session
- Individual therapy sessions: $175 for a 50 minute sessions
- Credit cards

- FSA/HSA
All payments are done through IvyPay for self-pay clients.
